Park Avenue Health and Rehabilitation

My mother resides in this facility and she loves it! It gives me peace of mind especially since I don't live in the same state as mymother. The staff is absolutely amazing! Very accommodating to my requests and my mother's needs. I feel at ease knowing she has this level of care. I would recommend this facility to anyone looking for a comfortable place for their loved ones. I am extremely satisfied knowing she is in good hands. Staff is always available to help me and answer my questions.

Type of care- First of all I wish everyday could have been Saturday and Sunday. Nicole is one of the finest CNA your facility has she was sweet caring and I just think she is over worked and under paid. Silva came in on Friday I only saw her that day. Monday she was back and she is very good also. My nurse was Kevin he is the only nurse I saw more than just to give me medicine. He helped me to get in and out of bed. He would come into my room to see if I needed anything. Now let's on to the bad! THE FOOD IS AWFUL! I called it the mystery meals, you never got a menu, you couldn't choose your meal ... I did not eat the food for two days. My friend brought me in some meals. I had snacks I could eat. I never ever received a hot meal ever. This place is awful for the food 0 stars. The care is just as bad no one comes if you call unless it was Nicole or Silva CNA's. Those two do all the work while there are at least 4-5 nurses sitting in the nurses station saying I don't clean butts! WHAT?Now the BEST OF ALL PHYSICAL THERAPY AND OCCUPATIONAL THERAPY. FIVE STARS that part is exceptional!!! John was my PT he was just the best can't even say enough about him. Simone OT she helped me to get my teeth brushed and to the bathroom back in bed she did things a CNA would do. Also, I had another girl who I can't remember her name blonde Laura maybe she was also amazing. Jacob is the director of PT and he does the best job ever. Gave him all my complaints and he was there for me. Someone like me needs to go in under cover and see what is going on.

Avoid This Place at All Costs!If you care about your loved ones, do NOT send them to Park Avenue Health and Rehabilitation. This facility is a nightmare--neglect, incompetence, and outright disregard for human dignity run rampant here. My mother suffered in their so-called "care," and I wouldn't wish this experience on anyone.The staff is either overworked or indifferent, leaving residents to fend for themselves. Basic hygiene? Ignored. Proper medical attention? A joke. Communication with families? Nonexistent. It's heartbreaking to see people treated like burdens instead of human beings who deserve respect and compassion.This place should be shut down. If you're considering Park Avenue for a loved one, RUN in the opposite direction. They don't provide care; they provide suffering. Shame on them for what they put my mother through.This place along with Haven Health, also known as Haven Hell, are complicit in my mom's death in 2022..

My Mother was placed at Park Avenue Rehab on September 3,2024 She Fractured Her Left Hip and needed to get mobile again physical therapy they promised to take care of her We would come visit her almost every day she seems scare but my mom can not commit very wellI call every week to see how her progress was on Never did they give me a call back. On September 17,3024 The nurse they where having trouble with her feeding tube so the took her to UMC Tucson where the said they fix Park ave sent my mom There time stating her feeding to was not working again So they took her to UMc Tucson ER and demanded she get a G-tube Place or They where not take her they left her in the ER. The UmC Tucson ER notice Serious signes of neglect she was dehydrated her legs where in traction and she couldn't not move them That mean They never did anything Physical Therapy on her at all so the reported they To adult protective services please don't ever have a Loved one stay there There is a lot more but I don't have alot of room to write very thing Down This really Happen to my Mom

I don't usually write negative reviews but this was the worst nursing care we have ever experienced. I would come to visit my husband no later than 9:am. I would walk into 4 full urinals . Ask nursing why aren't these emptied ? They said he has to turn his light on. Next issue call lights could be on for 25 + minutes. The nurse told me if we need anything come to the charting room and get her. Really One night midnight shift my husband needed to use the bathroom. The CNA told him NO . You either use a bedpan or a diaper. She had 38 patients. She was working by herself didn't have time. Linens where stained and had holes. Couple times my husband didn't receive his meal. So they brought sandwiches from somewhere that wasn't fresh by far. Therapy was less than 30 minutes. My husband was in for a fractured hip that wasn't operated on. Needed therapy. Jefferson Was his PT . We really appreciated what time he did spend with my husband. He was overwhelmed also. He never complained and truly loved his job. Sign out side say we are expanding hiring RN LPNs and CNA . They just plain are sort staffed. The staff they did have didn't enjoy there job.

A wonderful facility that provides exceptional care. The team specializes in therapy, IV and complex wound care. Would definitely recommend this facility if post acute care was needed for a family member.
