So I came from TMC after a 3 wk stay.....I was transported in this little 'mini van' type pt transfer thing---when getting on the guerney in the hospital numerous times I asked the man to pls put on the brakes--he said 'no it's fine I got it' and then while en route my personal walker started to fall on me--he had to stop and put on floor.....he ran two red lights on our way over here---and when he was putting me in my room (mind you no one treated us at all on our arrival--he said. 'You should not have come here--they ignore everyone and this place has a bad rep'. He was right. I was in the room by myself for like 45 min when I pressed this weird looking plastic button thing and a girl from admissions came in--and asked if I needed something. She said she was unaware that I had been transferred. Took info and had me sign a few papers.....then left. The room smells like pee. It is dreary and just pathetic. Old stuff old furniture....bathroom sink leaks as there is water on floor which to this moment ( and I reported it) has not been fixed for the last 3 hours! I was lucky I asked for a bottle of water otherwise I would have nothing!! I laid down as I was tired and fell asleep.....then pushed the call light and a rn came in and she said can you pls push the button on the wall to turn off? I said no because I have spinal fractures and I can't reach nor turn like that. Also the string that apparently turns off the overhead light is on the other side of the bed where again I can not reach.....this place is horrific--do not waste your time coming here as it's disgusting. Now to the food--sandwhiches/pre packaged salads and just entirely gross!!! I am on a special diet so that means most of all they have here I can't and won't eat!!

My mom had a fall and had to have her ankle replaced. After surgery the doctor recommended she go to a rehabilitation facility, because she had to be completely off her foot for 6 weeks. I was given a list from the hospital that I had to pick a place for and was given less then 24 hours. I took the list and researched as best as possible. I (unfortunately) chose Catalina Care. I was not able to meet my mom there, understandably due to COVID. I had to have an appointment to see her. I called and spoke with Ethan and he's scheduled my appointments for the week. When I arrived for my first visit, the receptionist told me she had to check and make sure m mom didn't have any one else in her room. (You would think she would of had it on her appointment list). She left and came back out holding a patient water pitcher, went to the water cooler, filled it and went back to the patient area. She returned and showed me where my mom was.My mom had pushed her button at 9:30 am because she needed water and just got water before I came in. It was 11:00! My mom also told me they are keeping her in a diaper 24/7. She would push her button because she needed to be changed, no one would come to see what she needed for hour's. After 2 days there, she developed a rash. She would ask for cream to help with the rash and she would be ignored. The nurses/staff were not friendly or caring. The place was not very clean and reeked of urine. I immediately started looking for a new rehabilitation facility. My mom is fortunate to have a family advocate, some of these poor patients are not so lucky. DO NOT PUT YOUR FAMILY MEMBER IN THIS FACILITY. I WOULD NOT LET MY DOG STAY IN THIS FACILITY. THEY NEED TO BE SHUT DOWN!!Thank goodness I got my mom out and she is thriving at her new place. I have an open Elder Abuse case against this facility.I hope this saves at least one person from being placed here! If I could give negative stars I would.

There is only one reason why this review is going to be two stars, so I will begin with that... the staff is extremely nice. Sadly, an individual's kindness, does not equate to their competency.When you walk in to one of the rooms with you sick loved one, you feel like you are walking into a monastery. The walls are so dark, and there is hardly any light, there is no color, no decoration... you are sitting there wondering horrified that your loved one is going to be stuck staring at these walls for weeks and just imagine the patients turning into cave dwellers. What happens next shows most of this facility's incompetency. The nurse who is checking my family member is is informing us that there is a set menu and that it doesn't change. We said that was fine, but informed her that my family member is allergic to red meat. The nurse said that was fine and that she would put it on the notes. Between my mother and I, that night we made sure we told three different individuals to make sure her dietary needs made it onto her chart. I came in the next day to provide entertainment for my family member, and the lunch cart comes in with meatloaf.... I yet again informed the staff that she cannot eat red meat and yet again I am told that they would add it to her charts. As you can imagine, it got to the point that I had to tell the staff seven times about her dietary needs. Nothing changed until I spoke to the social care worker and she got me in contact with the head dietitian. The head dietitian actually put on my family member's chart and finally told us that there is a menu that is given to patients the morning before to have some say on what they want to eat the next day. What we were finally told, was a process they already had set in place, that should have been told to us as soon as we walked in the door. We then realize that we were not told anything at the beginning. We were not told when my family member's physical therapy would be, when they would get a shower, when they would have the bed linens changed. My family member was put in the same bed linens they had at the hospital into the rehab center and we had to ask them to be changed because they were left on her for four days and without her even being wiped down. When my family member was taken out for physical therapy, they could have easily changed out her linens, but instead they just tucked in the blankets to make it look like they changed the bedding. We were also told that between shower days we could ask for a basin for patient's to wipe down. We asked for that, but were never given one. My mother and I had to bring in everything to wipe our loved one down.Probably the worst thing was the amount of follow up given by the management. In the middle of the dietary incident I asked to speak to the superintendent. Either he was never told, which is a poor statement on the staff, or he never cared to address one of his patient's concerns. Then later when they went to discharge my family member, they only spoke to the patient (oh and even in their notes it mentions that this patient does suffer from dementia). We tried to call them to get the information as to why they were being discharged and they never answered the phone, nor called us back.There is a possibility that if you are here, you have no choice in coming here. First if you have a choice, don't come here. If you do not have a choice, then be prepared to be there daily to take care of your loved one. After this experience, I do not trust them to dog sit, let alone take care of an ill loved one.

I was there in September. The staff was great especially the physical therapy staff! The cafeteria food was excellent as well! I appreciate the care I was given during my rehab recovery.

The majority of the staff is absolutely wonderful and has a wonderful work ethic. They are understaffed and need more help on the floor to give patients proper care. Patients turn on their call light and can wait anywhere up to 1.5 hours for someone to come to see what the need. I feel that's an unexpectedly amount of time. What if someone was choking..?..they would be dead. I consider that unacceptable. Communication on services and the ability in how to access what's available to patients also seems to be a major issue. Overall, they have a lot of work to do for a higher rating.

My father was sent to this facility straight from the hospital and it was the most awful experience I've ever seen. From the dark tiny overcrowded rooms (3 people in a very tiny space) to the lack of care and concern, it was like he was in a prison in solitary confinement. We could not wait to transfer him to a different facility for rehab. If I could rate this facility a negative number, I would.

If I could give a negative stars I would. This place has awful care quality. They don't answer the button. They treat the people who are there horribly. They don't keep up on treatment at all. Leaving people to sit in diapers all day long. Call for a nurse no one shows up for hours. If you try to talk to staff some of them are so rude and disgusting. You can tell they are there for a check. People would heal better being treated by crackheads than the care they receive at this facility. If the care my friend is getting continues we will be pressing the issue with the law. They thought she was stupid and couldn't understand them. Guess what. You guys were wrong! She's been signing to me about the abuse. Hopefully posting this will make the abuse stop. Staff running people into walls on gurneys and laughing about it. Disgusting the place smells like death and shit. Broken furniture. My friend is a fall risk and her bed doesn't even have rails. especially if you're black, don't come here. We will pinpoint which staff members she said abused her and press charges against this place. Horrendous, needs to be shut down.

The physical and occupational therapy staff is great. They are friendly, patient, and kind. Although we feel like they need to have more staff so they can do more therapy with the patients. The doctor was also very good. Concerning the nursing staff and care givers, they have too many patients to care for so their availability to help patients is limited. But we feel that the important thing in a patients recovery is the attitude of the staff members. At Catalina it depends on who is on shift at the time you need them. About 60% of the nursing staff was friendly, kind, and patient, about 40% were dismissive, impatient, and gruff. About 40% of the care givers were friendly, kind, and patient, about 60% were dismissive, impatient, and gruff. There were times when assistance was needed to get to a bathroom but on one would respond to the call button. In one instance the call button was pushed, which illuminates a light above your room door in the hallway, and no one came all night long. This facility has some serious issues with the attitude of many of their staff members and a shortage of staff to meet the needs of their patients. Another problem is that they are putting three patients in rooms designed for two beds.
