My mom was transferred there on a Monday afternoon at 3pm I took her out Thursday morning. She had been in hospital for approximately 3 1/2 weeks. She had surgery. She's 79 years old. She had a colostomy bag and foley bag. She was there for rehab to build her strength back up. If I could give them 0 stars I would. I have not one good thing to say about that place. The care is horrible. In my opinion there's absolutely zero correlation about what care a new arrival needs between the workers. My niece,sister and I are the one's who changed and cared for our mom. It took them 3 days to bring her clothes to her room which consisted of 3-4 pairs of pajamas and 1 pair of jeans. In my opinion horrible place. I would never recommend taking a loved one there for any reason. Yes I absolutely plan on turning them in to state!!

The service isn't up to par. My family member seems to get Baer minimum care. Member calls me often with issues not being cared for.Choose of another food beside what is served, baths, walks, physical therapy - must wait for insurance approval. If workers walking patients/residents: they wouldn't need physical therapy every couple of months

My husband has been there 2x. He likes it. Friendly staff. Responsive to his needs, they took great care of my husband. And he could smoke in the inner courtyard. Not that any doctor would approve but it did help his mood.

I had a family member here and they did very little to help with the physical therapy and rehabilitation process. If they were tired, the staff did not make them do therapy at all. Many times I had gone in and found that staff had not gone in and checked on my family member. When I would walk past the PT/gym area, I rarely saw a patient in there, staff would just be in there hanging out. The one time I did ask for help, I was told that a second staff member was needed and they were on a 15 minute break, I did not get any assistance for 45 minutes. The inside of the facility looks nice when you walk in, but that is the only nice thing about this place. If this were the last place in Nebraska, I would not recommend anyone taking their family member here for any reason.
